<hr width="25%"><em>Evaluation of a topical treatment containing 8% sulfur and 2% resorcinol in the treatment of centrofacial rosacea</em></p>

<p>A combination topical treatment containing 8% sulfur and 2% resorcinol has been shown to be an effective treatment for acne vulgaris. To evaluate whether this combination drug is also helpful in rosacea, we recruited 15 female volunteers, aged 43 to 52 years, with classic centrofacial, erythematotelangiectatic rosacea. Ten subjects received the active treatment bid for 3 months and 5 subjects received vehicle for the same period. A number of bioengineering methods were used to assess facial skin at baseline and at 2, 4, 8, and 12 weeks. In the active group digital photography and videomicroscopy showed moderate reduction in the appearance of telangiectatic vasculature. Colorimetric measurement showed a small reduction in redness; blood flux imaging by a laser Doppler device showed decrease in blood flux. Compared with this, the control group (vehicle-treated) either showed no difference or worsening of rosacea posttreatment. The global assessment by the subjects also indicates the improvement of rosacea in the active group.  <br />Author disclosure: Nothing disclosed at press time. <p><strong>BACKGROUND:</strong> Immunoglobulin (Ig)E-mediated hypersensitivity is a mechanism suggested to explain adverse reactions to buckwheat. This is the first reported case in the United States of a person who developed asthma and worsening allergic rhinitis after exposure to a buckwheat pillow.</p>
<p><strong>OBJECTIVE:</strong> To describe a patient who developed asthma and worsening allergic rhinitis after exposure to a buckwheat pillow and to provide evidence that the adverse reaction was IgE-mediated.</p>
<p><strong>METHODS:</strong> The patient underwent skin prick and ImmunoCAP testing (Pharmacia Diagnostics, Kalamazoo, MI) to buckwheat as well as skin prick testing to several environmental allergens. RESULTS: The patient showed a 4+ skin prick test response to buckwheat. He also showed 4+ positive skin prick responses to multiple trees, grasses, and weeds, Alternaria, Helminthosporium, dog, and histamine control and was 3+ positive to house-dust mites, Penicillium, Aspergillus, cat, and feather mix. His negative control was negative. His ImmunoCAP test for buckwheat-specific IgE was class 4, or strongly positive. He had normal spirometry values. Performance of house-dust mite avoidance measures did not result in improvement of the patient&#8217;s symptoms. Removal of the patient&#8217;s two buckwheat pillows resulted in resolution of his asthma and improvement of rhinitis symptoms.</p>
<p><strong>CONCLUSIONS:</strong> The positive skin prick and ImmunoCAP test to buckwheat along with the positive clinical response to buckwheat pillow elimination support an IgE-mediated mechanism in explaining our patient&#8217;s buckwheat pillow-induced asthma and allergic rhinitis.</p>

<p>Dr. Darm is known for his usage of Photoderm and also his Genesis Laser, using a different approach compared to other well known Photoderm and IPL practitioners.</p>
<p>Recently Dr. Darm has developmed a new three-pronged protocol to treat rosacea. This protocol involves a compressed treatment period, forcing a flush during treatment session and the usage of the genesis laser on the whole face.</p>
<p>The development of a compressed treatment protocol has raised some interest by rosacea sufferers. Concentrating treatments over a shorter period of time seems to allow more clearing sooner. Treating more often than the usual 2-3 week interval means fewer trips to Oregon, quicker resolution, and the theoretical advantage of reduced angiogenesis (regrowth of new blood vessels).</p>
<p>The second interesting part of Dr. Darms treatment protocol is the aggressive induction of flushing during treatment sessions.</p>
<p>Finally the third unique part of his treatment protocol involves the use of the genesis laser which adds another wavelength to the treatment sessions (1064 ND YAG).</p>
<p>In the period between treatments, Dr. Darm likes periostat to help with the inflammation. Periostat has the advantage of being well tolerated and unlikely to introduce antiobiotic resistence problems.</p>
